EMR, EHR, PHR: What's the Difference?

Electronic Medical Record (EMR)

  • Records maintained by a single healthcare provider.
  • Replaces a paper chart

 Electronic Health Record (EHR)

  • Records from many providers shared across the continuum, e.g.,  your demographics, allergies, medical history, lab reports, immunization records, radiology, prescription medications, etc.
  • This is what the government is challenging providers to create

Personal Health Record (PHR)

  • Records created and updated by you
  • Example:  your diet or exercise plan and progress
