Electronic Medical Record (EMR)
- Records maintained by a single healthcare provider.
- Replaces a paper chart
Electronic Health Record (EHR)
- Records from many providers shared across the continuum, e.g., your demographics, allergies, medical history, lab reports, immunization records, radiology, prescription medications, etc.
- This is what the government is challenging providers to create
Personal Health Record (PHR)
- Records created and updated by you
- Example: your diet or exercise plan and progress
